Slough Borough Council is seeking an experienced provider for the delivery of an Integrated Health and Wellbeing Service for residents of Slough.
The core purpose of the service is to improve the health and wellbeing outcomes of Slough residents by implementing an Integrated Health and Wellbeing Lifestyle Service, including the following elements: Smoking Cessation, NHS Health Checks, Alcohol Brief Interventions, Falls Prevention, Oral Health, Sedentary Behaviour Change, and Tier 2 Child and Adult Weight Management. This service will enable residents to take responsibility and control of their own care and enhance the collaboration between the Local Authority and Health services at a local level.
The provider will need to demonstrate that they have the knowledge, innovation and ability to deliver the services that meet the needs of a diverse population.
Contract start date is the 1st April 2020 and will be for a term of three years with a possible extension of one year, plus one year.
The financial envelope available for this service is £720,000 per annum.
